Acceptance of Privacy Policy


Introduction


This privacy policy (“Privacy Policy”) describes how Choose Your Horizon Inc. (“Choose Naltrexone”, “us”,  “we”, “our”, or variants) collects and uses information about you through the Site, email, text, and other communications between you and Choose Naltrexone.

The Privacy Policy refers to information we collect:

    • from on our Site
  • from or on behalf of your medical providers and their staff
  • through email, text, and other electronic communications between you and our Site
  • when you interact with our advertising, marketing, and applications on third-party websites and services, if those applications or advertising and marketing include links to this policy.

The Privacy Policy does not apply to information we may collect:

  • offline or through any other means, including on any other website operated by Choose Naltrexone or any third-party
  • through any third party website or application, including through any application or content (including advertising or marketing) that may link to, or be accessible on or from, the Site.

Choose Naltrexone provides managed services to independent provider groups. Choose Naltrexone is not a provider group itself, has no ownership interest in the Medical Groups, does not have control over the policies or practices of the Medical Groups, and is not responsible for the quality of care provided by the Medical Groups. The Medical Groups have their own separate "Notice of Privacy Practices", which can be found on the Site.

Please read this Privacy Policy carefully to understand Choose Naltrexone's policies and practices regarding your information and how we use it. If you don’t agree with our policies, you are not permitted to use our Site. Therefore, by accessing or using our Site, you agree that you have read this Privacy Policy and you understand, and consent to be bound by, the terms and conditions herein. This Privacy Policy may change from time to time. Your continued use of our Site after we make changes shall constitute your acceptance of those changes, so please check this Privacy Policy periodically for updates.



Information we collect about you


We receive and collect information that you directly submit to us or that we gather when you use our Site, including personal information such as your name, mailing address, telephone number, email address, health data, and payment information, as well as non-personal information such as IP addresses, information that is passively or automatically collected through cookies, your browser or device, or other services, and information from third parties. We also collect information on your physical location that may be provided by you or collected through the use of our Site or other communication channels. In this Privacy Policy, we refer to all of this as “Personal Information.”

Information From Automated Data Collection; Cookies and Trackers

We receive certain information about you through companies that provide us with such information as part of their relationship with us, including Meta, Google Analytics, Google Ads, Hotjar, Segment, Microsoft Bing Ads and other tools. These organizations may automatically log device data, online activity data, and geolocation data.

  • Analytics - Our Service works with analytics companies that track online activity to understand the number of unique users visiting the Service and to understand how users are navigating our Services.  
  • Advertising - We also work with advertising networks that may track you to measure the effectiveness of the advertisements so that personalized advertisements or offers may be made for you. These third parties may use cookies, web beacons, device identifiers, and other technologies to collect information about your use of our Services and of other website and mobile apps – that information may include your IP address, web browser, mobile network information, pages viewed, actions completed on the website and in our mobile app, time spent on pages or in mobile apps, links clicked, and conversion information. This information allows us to determine whether you have visited the Service previously, remembers preferences that you have set, statistically monitors how many people are using our Service and for what purposes, and allows delivery of advertising targeted to your interests. When you purchase a service from the Choose Naltrexone website, web beacons, pixels, or API from our service providers’, advertising providers’, and marketing providers’ analytics tools may capture and transmit the conversion data to help assess the efficacy of advertising campaigns and inform future advertising.
  • Session replay technologies - We use software code to track users’ interactions with the Service; the replays include users’ clicks, mobile app touches, mouse movements, and scrolls during those sessions. These help us with website optimization to understand how users are engaging with the website.  


How we use your information

  • enforce our rights arising from contracts or otherwise

Marketing, advertising, and analytics.  

We and our advertising providers may collect and use your Personal Information for marketing, advertising, and analytics purposes:

Interest-based advertising. We engage advertising companies and social media companies that use cookies and similar technologies to collect information about your interactions over time across the Service and other websites, and use that information to serve online ads that they think will interest you.  These include, but are not limited to:

  • Meta Platforms. We use Meta Platforms (including, Facebook and Instagram) (collectively, “Meta”), a web analytics and advertising service provided by Meta, on our website. The data from Meta is also saved and processed by Meta. Meta may connect this data with your Facebook or Instagram account and use it for its own and others advertising purposes, in accordance with Meta’s Data Policy which can be found at https://www.facebook.com/about/privacy/.   • Google Analytics and Google Ads. We use Google Analytics, a web analytics service provided by Google, Inc. (“Google”) to collect certain information relating to your use of our website. We also use Google Ads, an internet-based advertising service, to advertise our services and analyze the performance of our advertising campaigns. Creating anonymized, pseudonymized, aggregated, or de-identified information.  

Your Personal Information may be anonymized, pseudonymized, aggregated or de-identified by removing data components (such as your name, email address, or linkable tracking identifier) that makes the data personally identifiable to you. We do this for our business operations, to disclose to other parties for our lawful business purposes, and for research and development of new products or services.



Disclosure of your information


We do not sell, share, or otherwise disclose your Personal Information for reasons other than those described in this Privacy Policy.

We may disclose your Personal Information to a few third parties, including:

Healthcare parties, including the Affiliated Medical Groups (CYH Medical Group, ( DE) P.A., CYH Medical Group of California, CYH Medical Group TX, CYH Medical Group of NY) and other entities that assist with treatment, payment, and other operations so that they can deliver healthcare services to you and conduct related processing and billing functions.

  • your healthcare or other licensed medical providers
  • our parents, subsidiaries and affiliates
  • our third-party service providers, contractors, subcontractors, and business associates we use to support our business
  • to any company we might merge with or acquire, or that acquires us, or in the event of structural change of our company of any form (e.g., a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Choose Naltrexone's assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding).

We may also disclose your Personal Information:

Notwithstanding the foregoing, we may also disclose, without restriction, aggregated information about our users and information that does not identify any individual user.


Accessing and changing your personal information


You can access, review and update or change your Personal Information at any time either through our site, if available, or by notifying us through the channels outlined in this Privacy Policy. We may not be able to accommodate your request, however, if we believe it would violate any law or legal requirement or cause the information to be inaccurate or fraudulent.

  • Sensitive Personal Information Uses or Disclosures: You have a right to limit our use of “sensitive” Personal Information to purposes specified in the CCPA.
  • Business or Commercial Purpose for Collecting and Selling Information: We collect Personal Information for the business and commercial purposes outline above.
  • Categories of Third Parties with Whom We Disclose Information: We may disclose your personal data with third parties as described above.
  • Sale or Share of Personal Information: Because we engage in the practice of cross context behavioral advertising, we may “sell” and/or “share” your Personal Information as those terms are defined by the CCPA. In the preceding 12 months, we may have “sold” or “shared” identifiers, commercial information, and internet or electronic network activity information with data analytics, advertising networks, and/or social media networks. Under California’s “Shine the Light” law, California residents are entitled to request and obtain from us once per calendar year information about the categories of Personal Information we have shared with third parties for their own direct marketing purposes. Notice to Washington and Nevada Residents

If you reside in Washington or Nevada, please read this section for additional information about how we collect, use, and disclose Consumer Health Data, defined below, under the Washington My Health My Data Act and Nevada Consumer Health Privacy Law.

Depending on your relationship with us, some of the Personal Information that we collect about you may identify your past, present, or future physical or mental health status (“Consumer Health Data”).   • We do not “sell” Consumer Health Data or permit third parties to collect Consumer Health Data over time and across different internet websites or online services, unless we obtain your valid authorization.


Choose Naltrexone does not sell, rent, or lease your Personal Information to third parties according to 2019 Nev. L. 211. Choose Naltrexone strives to use industry-standard data collection, storage, and processing practices and security measures to protect against any unauthorized access to, accidental loss of, or disclosure of your information.These safeguards may vary based on the sensitivity of the information that we collect and store.


Unfortunately, no security method or combination of methods is fool proof. We will always strive to do our best and to work with third-party service providers that strive to do the same, but we cannot warrant or guarantee the absolute security of any Personal Information that may be transmitted to or from our Site. There is no guarantee that data may not be accessed, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards. Therefore, any transmission of Personal Information is at your own risk. We are not responsible for circumvention of any privacy settings or security measures contained on the Site or your computer or mobile device.

Children under the age of 18


This Site is not for persons under the age of 18 (collectively, “Minors”). Minors are not authorized to use this Site or provide any information through the Site. We do not solicit or knowingly collect information from Minors, and if we become aware of any such information being collected, we will take immediate action to delete it.
